To Qualify For B1 Guarantee...

1/ Use the Boutique Whitening consent form and ensure it is signed.

2/ Take time stamped photos prior to whitening, where shade tab and notation on the tab are clearly visible.

3/ Use the super-seal custom trays from Boutique Whitening.

4/ Use the full 8 syringe B1 kit at 16% concentration.

5/ Take time stamped pictures after whitening has been completed,where shade tab and notation are clearly visible.

6/ The photos must be no more than 12 weeks apart to claim on the guarantee.
